I hereby grant permission to DermResearch Inc. for the use of the photograph(s) or electronic media in order to determine my potential qualification for a clinical trial. I understand my image will be evaluated by a dermatologist and that my uploaded image will be used to determine either a 1) high or 2) low probability of having the right skin condition for a clinical trial and I consent to this evaluation and understand this evaluation is not a medical diagnosis, is not considered medical care, and is not a consent to participate.
I understand this evaluation does not guarantee participation in a clinical trial and that in order to participate, I must still attend an in-person screening visit and meet inclusion and exclusion criteria at the time of my screening visit.
I further certify that I am an adult at least eighteen years of age, or alternatively that I am the legal parent or guardian of the applicant and consent to these terms and conditions on their behalf.
I understand that I may revoke this authorization at any time by notifying [email protected] in writing. The revocation will not affect any actions taken before the receipt of this written notification. Images will be stored in a secure location and only authorized staff will have access to them. They will be kept until evaluated by a dermatologist and will be destroyed immediately after this evaluation.
